Cosmetic rhinoplasty is what we most often associate with the term “nose job”. This procedure often involves reducing the size of the nose, narrowing the nose, and/or altering the position of the nasal tip. It is performed for aesthetic reasons. When rhinoplasty is performed for the first time, it is called “primary rhinoplasty”.

Functional rhinoplasty

Functional rhinoplasty is performed to help a patient breathe more easily or reduce snoring. This procedure may involve a variety of maneuvers designed to improve the airway while preserving or improving shape. Dr. Holcomb or Dr. Salzman will perform the functional components of a rhinoplasty as part of a cosmetic rhinoplasty procedure.

Tip rhinoplasty

Tip rhinoplasty addresses only the nasal tip, which often means that the recovery time is shorter than a regular rhinoplasty. In this procedure, the nasal tip is altered to achieve a better projection and balance with the rest of the nose and face. The bone structure of the nose is not affected in tip rhinoplasty.

Revision rhinoplasty

Revision rhinoplasty is done to correct a primary rhinoplasty. It is often very challenging because it involves three-dimensional contouring in the face of scar tissue that is present from a first surgery (or multiple surgeries). Revision rhinoplasty may require removing tissue where too much remains, adding tissue grafts or implants where too little volume exists, correcting asymmetry, altering tip position, and/or narrowing the base.

Rhinoplasty is most often performed under general anesthesia, but is also occasionally performed under IV sedation. It is typically an outpatient procedure and can take between 1 and 3 hours. Dr. Holcomb and Dr. Salzman generally utilizes the “open rhinoplasty” approach, wherein the incisions are made in the columella.

From here, the tissue and skin can be lifted and separated from the underlying structures of the nose so that Dr. Holcomb and Dr. Salzman have better visualization and attention to detail. Although most often open rhinoplasty is chosen, closed rhinoplasty can be performed as well. In closed rhinoplasty, the incisions are made on the inside lining of the nose.

Dr. Holcomb employs advanced techniques for optimal contouring results. Modern structural support emphasizes a well-supported, yet naturally contoured, nasal tip. He prefers caudal septal preservation for maximum tip support and, when necessary, may utilize a septal extension graft to ensure balance.

Achieving a natural, flowing contour from the upper nose to the tip and base is paramount. This requires attention to the middle vault width. When the middle vault is too narrow, subtle widening using spreader grafts can improve both aesthetics and airway dynamics.

Rhinoplasty Recovery Timeline and What to Expect

Initial recovery progresses quickly, with many patients feeling comfortable enough to return to work or school within 7 days. However, it's essential to allow for 2 to 3 weeks of social downtime before resuming vigorous physical activity.

Bruising usually resolves within two weeks, while swelling decreases over the first few weeks. Some residual swelling can take up to 12 months to fully subside.

A nasal splint is worn during the first week to stabilize the nose. For 6 weeks, avoid any accidental trauma to the nose to ensure proper healing of delicate nasal structures.

Swelling above the nasal tip takes the longest to resolve, gradually reducing over 3 to 6 months. Strenuous activities should be avoided for 4 to 6 weeks to ensure optimal healing and final results.
