Information for Out of Town and Out of Country Patients

Dr. Holcomb, Dr. Kreithen, Dr. Salzman, and their staff happily welcome out-of-town patients, and we will do everything possible to accommodate your needs during your visit.

Electronic medical records

Prior to your consultation, we will need you to complete your medical history using our secure, HIPAA-compliant patient portal so that we can update our electronic medical records for you. You will receive instructions on how to do this after your consultation has been scheduled.

Flying In

Dr. J. David Holcomb, Dr. Joshua C. Kreithen, Dr. Salzman, and their staff welcome out-of-town patients and will strive to accommodate your needs during your visit. We have completed our transition to Electronic Medical Records – prior to your consultation, we will need you to complete your Medical History using our secure, HIPPA-compliant patient portal. You will receive instructions after your consultation has been scheduled.
